We are excited to announce that Cozeva is among California’s 9 organizations recognized by the California Health & Human Services Agency (CalHHS) and its Center for Data Insights and Innovation (CDII) as a Qualified Health Information Organization (QHIO).
The Data Exchange Framework (DxF) is California’s first-ever statewide data sharing agreement across health and human services systems and providers to share information safely. That means every healthcare provider can access the information they need to treat California’s patients quickly and safely.
DxF is key to achieving:
DxF ensures that providers can access all the information they need to provide safe, effective, whole-person care through the secure exchange of health and social services information.
The DxF facilitates a secure exchange of health and social services information for all Californians, bridging and understanding gaps on social determinants of health. This empowers providers and services to tackle inequities, particularly in historically underserved and underrepresented communities.
The DxF will modernize patient experience through better access to information that health and social services providers need at the point of care. The DxF also eliminates patients’ sole responsibility to maintain and carry their own health records, creating seamless interactions with healthcare providers.
Health and social services providers are joining the DxF in a shared commitment to provide safe, effective, whole-person care and creating a Healthy California for All.
A QHIO is a data exchange intermediary that meets the criteria for secure exchange of health and social services information. This ensures timely access to crucial information to all health and social services providers for effective treatment of California’s patients. A QHIO must comply with the terms and conditions of the Data Sharing Agreement (DSA).
The DxF Data Sharing Agreement (DSA) is a signed agreement and shared commitment between hospitals, physician organizations and medical groups, skilled nursing facilities, health plans and disability insurers, clinical laboratories, acute psychiatric hospitals, and other voluntary signatories to quickly, securely, and appropriately share information, while following a common set of terms and conditions set forth in the DxF Policies and Procedures (P&Ps).
