Boost Your Patients’ COVID-19 Immunization Rates with Cozeva!

Boost Your Patients’ COVID-19 Immunization Rates with Cozeva!

April is National Minority Health Month with the theme of Give Your Community a Boost! The U.S. Department of Health and Human Services Office of Minority Health is focusing on continued efforts to vaccinate those eligible due to COVID-19 as the third leading cause of death in 2020 (CDC). It’s also vital to examine “underlying health and social inequities that put many racial and ethnic minority groups at increased risk of getting sick, having more severe illness, and dying from COVID-19.” (CDC)

COVID-19 Vaccine and Health Equity Issues

A variety of factors impact vaccination access and acceptance, which commonly affect racial and ethnic minority groups. These factors include education, income, job access and working conditions, racism, health care access, transportation and neighborhood conditions, and lack of trust. (CDC).

Immunization rates among racial groups have been shifting with the on-going efforts to immunize those still unvaccinated or have yet to receive booster shots. The CDC data at the national level shows some significant gaps to understand who is and is not getting vaccinated. (KFF)

Use Cozeva to Manage Your Patients’ COVID-19 Immunizations and More!

As a population health management tool, Cozeva supports plan administrators, provider organizations, and their practices to easily track their patients’ COVID-19 immunization care opportunities. Outreach to those still not fully vaccinated or needing boosters is key to fighting this ongoing pandemic.

As a real-time metrics engine, Cozeva’s gap reports and powerful Analytics can better inform your organization’s outreach efforts. With options to target patients by geographic area, by length of time from last service/immunization, or by available race, ethnicity or language data, these efforts can improve patient compliance and engagement for improved health outcomes.

Furthermore, our team is working on incorporating Social Determinants of Health factors as part of a patient’s care opportunities. Stay tuned for this exciting development to address whole person care and challenges to health equity!
